NOTICE OF TERMINATION OF MEMBERSHIP

Dear Sir

I/We, ______________________________________________________________, a member

of Horizon Hills Golf & Country Club under membership no. _______________, would like to
terminate my/our club membership with effect from ________________.

I/We hereby agree that:

1. There shall be a one month notice period given to the Club to stop charging the monthly
subscription fee;
2. I/we shall settle all outstanding amount due by me to the Club prior to termination of the
membership;
3. I/we shall immediately return the following to the Club:
a. Memberhip Certificate;
b. All membership cards issued to me and my supplementary members;
c. Car stickers; and
d. Golf Bag Tags.
4. I shall pay any penalty imposed by the Club for non return of the above items.
5. In the event that the termination of my membership is due to transfer of membership, I
hereby undertake to pay the subscription fee up to the date the membership is activated by
my transferee.
