AFFIDAVIT FOR DELAYED REGISTRATION OF BIRTH

I, ELVIRA ALINDAO BALIGUAT, of legal age, married, Filipino and with residence
and postal address at Tucdao, Bongoyan, Borbon, Cebu, after having been duly sworn
to in accordance with law, do hereby depose and say:

1. That I am the applicant for the delayed registration of my birth;

2. That I was born on July 30, 1964, in Mohon, Talisay City, Cebu, Philippines;

3. That I was attended at birth by Lala, a neighbor, now deceased;

4. That my parents were CLEMENTE ALINDAO and PATROCINIA ADLAWAN,

who were married on July 30, 1964, at Talisay City, Cebu;

5. That I am married to VICENTE BALIGUAT;

6. That the reason for the delay in registering my birth was due to my parents’
negligence;

7. That I am executing this affidavit to attest to the truthfulness of the foregoing

statements and for all legal intents and purposes.
