PLEASE TAKE NOTE OF THE FOLLOWING:
1. After pre-registering online, kindly wait for the text message informing you of your scheduled date
of vaccination and please reply/confirm your schedule.
... OR, You can proceed to register at your respective Brgy. Health Centers or at the Municipal Gym
(Health Services Office).
Come only on your scheduled date of vaccination.
2. The child MUST BE ACCOMPANIED by an adult (parent/guardian).
The parent/guardian and the child will need to sign the informed consent and assessment form.
3. Ready your requirements for Vaccination.
The requirements are as follows:
a. Documentary Proof of Filiation
-any document bearing the name of the adult in relation to the child.
Any of the following:
• Certified True Copy of Birth Certificate of the child issued by PSA
• Certification from the LCR of the municipality where the child is registered
• Baptismal Certificate with the name of the parent/s
b. Valid ID cards of the accompanying parent/guardian and the child.
c. Medical Certificate to be accomplished by the Attending Physician/Pediatrician for those WITH
COMORBIDITIES.
VACCINATION SITES:
1. BENGUET GENERAL HOSPITAL (BeGH): FEBRUARY 23 & 24, 2022

 MARCH, 2022: Every Thursday and Friday.

 TIME: 8:00 am to 1:00 pm

2. CORDILLERA HOSPITAL OF THE DIVINE GRACE (CHDG): Every Wednesday, Saturday

and Sunday starting February 23, 2022.
TIME: 8:00 am to 2:00 pm
