Q: What are the sources of reservists?

• The following are the sources of reservists:

• Basic and Advance ROTC graduates

• Basic Citizen Military Training (BCMT) graduates
• Professionals/Degree Holders (PRC License) Commission in the Reserve Force
• Graduates of MNSA at NDCP
• Military Orientation Training (MOT)
• Incorporated Officers (Retired Military personnel)

Q: I am an existing PAF Reservist. However, I did not report for a long time. How can I
reactivate my Reservist status?
A: Report to 1st Air Force Wing Reserve (AFWR) located at 1st Air Reserve Center Building, Villamor
Air Base Pasay City. Bring a copy of your Enlistment Order, look for Director for Personnel, W1, ask
for Data Captured Form, fill it out and submit and ask for your Group assignment. You will be asked
to report at least once a month for accounting purposes.

C- PICTURE (WHOLE BODY POSTCARD SIZE)
D- PERSONAL DATA SHEET or RESUMÉ (PRDMIS if Active Enlisted Reservist)
E- AFFIDAVIT OF WILLINGNESS FOR COMMISSION IN THE RESERVE FORCE
F- PSA BIRTH CERTIFICATE
G- CERTIFICATE OF COMPLETION OF BASIC or ADVANCE ROTC/BCMT
H- COLLEGE DIPLOMA & TRANSCRIPT OF RECORD (AUTHENTICATED by SCHOOL
REGISTRAR)

I- COMELEC CERTIFICATE (For Elected Officials)

or APPOINTMENT ORDERS FROM THE PRESIDENT (For Appointed Officials)

J- PHYSICAL EXAMINATION REPORT (Note: the executive check-up or annual physical

exam results can be validated by The Surgeon General, AFP)

- Dental Examination (Panoramic X-ray)

- Urinalysis
- Fecalysis - NP Examination
- VDRL - HIV Test
- Hepa B - Blood Chemistry Exam
- Drug Test - Chess X-ray
- ECG - CBC w/ platelet
- EENT - Others: Identifying Marks and Blood Type
 K- COMMAND CLEARANCE (will be issued later; the following will be submitted with the
application which will be used for the subsequent issuance of this Command Clearance)

 Barangay Clearance
 Mayor’s Clearance
 Police Clearance
 NBI Clearance
 CFI/RTC Clearance

L- LATEST DRUG TEST RESULT from duly recognized drug testing center of DOH

Compile these documents in eight (8) folders with one (1) original and seven (7) authenticated
photocopy with signature of applicant on the left side of the documents and proper index tabs.
