AFFIDAVIT OF GUARDIANSHIP

I, EMMYLOU T. ALAWI, of legal age, Filipino, single with

residence at Looc Riverside, Estaka, Dipolog City, Zamboanga del
Norte do hereby depose and say:

1. That I am the common-law wife of VERGEL L.

GUILLANO;

2. That we have been living together as husband and wife

since 2013 until his death on December 6, 2019 at Looc,
Riverside, Dipolog City;

3. That in our union we produced two children, namely

XHANDREX MEKHIO A. GUILLANO, born on June 1,
2013 (6 years old) in Pagadian City and XHADOWX
MAXHUELLE A. GUILLANO, born on January 26, 2019
in Dipolog City (11 months old);

4. That being their mother, the aforementioned minor

children are under my care and custody;

5. That I have been their guardian since their birth until

the present;

6. That I am executing this Affidavit in order to attest to the

truth of the foregoing facts and for whatever legal
purpose it may serve.

IN WITNESS WHEREOF, I hereby set my hand this 12th

day of December, 2019, in the City of Dipolog, Zamboanga del
