DATES: Saturday 8 October 2011 to Saturday 22 October 2011 (3 matches) Saturday 5 November 2011 to Saturday 10 December 2011 (6 matches) Saturday 4 February 2012 to 3 March 2012 (5 matches) Finals Matches 10 & 17 March (if required 24 March 2012) (Note: for the Sunday competition the dates are one day later) Conditions you agree to by signing this form 1. You are a member of Eildon Park Tennis Club. If NOT you need to join and pay before the season starts 2. You will pay ball money with this form ($35). 3. You note you are required to purchase and wear a EPTC t-shirt for matches 4. If you are selected for a team and then you withdraw, you lose your ball money, pay team the entry fee and jeopardise your placement in a team in the following season. 5. You are 18 or under as of midnight 31st December 2011. 6. Team selection is based on a combination of past performance, tennis ability and desired team combinations of ALL players. NOT ALL REQUESTS CAN OR WILL BE SATISFIED. 7. If you cannot play on a rostered day, it is your responsibility to swap with someone else. 8. You will pay any fines incurred by the club because of your failure to find a replacement. 9. Your parents agree to do their rostered duty. 10. Your behaviour will be appropriate to the rules of the Eildon Park Tennis Club. 11.
