PROJECT QR CODE CARES

I. Project Title: Quick Response Code for Convenient Arrangement and Riskless
Engagement in School (QR Code CARES)

II. Beneficiaries: Parents, Teachers, School Staff, Stakeholders, Visitors and Learners

III. Rationale:

Anchoring to Republic Act No. 11332 which is entitled “An Act Providing Policies
and Prescribing Procedures on Surveillance and Response to Notifiable Diseases,
Epidemics, and Health Events of Public Health Concern, and Appropriating Funds
Therefor, Repealing for the Purpose Act No. 3573, Otherwise Known as the “Law on
Reporting of Communicable Diseases”, supporting Republic Act No. 10173, otherwise
known as the Data Privacy Act of 2012 and pursuant to DepEd Order No. 14, s. 2020
“Guidelines on the Required Health Standards in Basic Education Offices and School,
this office designed project to be used for easy access, safety and security, contact
tracing, SBM, teachers and learners’ attendance and punctuality. As well as the number
of people that gain access to school for each day and for the future face to face classes.

QR code is a code that is read by a QR code scanner. Once scanned, all the
information, date and time of users who visited to the school premises will be recorded.
The collected data can be used to identify visitors, stakeholders, parents, teachers,
school staff and learners who enter in school or even in contact tracing.

IV. Objectives:

This project aims to:


⮚ Provide easier access and smooth collection of data to avoid congestion in
information area.
⮚ Monitor efficiency during distribution and retrieval of modules.
⮚ Identify, notify and track individuals who may have had close contact with a person
having confirmed or probable case of an infectious disease as a means of
controlling the spread of infection.
⮚ Implement the IATF Health protocols and guidelines.
⮚ Ensure safety and security of individuals.
⮚ Instill discipline and punctuality among the learners, teaching and non-teaching
personnel.
⮚ Promote best practices to other school that will lead to a good partnership to
provide innovation for school development process.

ACCOMPLISHMENT REPORT ON PROJECT QR CODE CARES

I. PROJECT QR CODE CARES (Quick Response Code for Convenient


Arrangement and Riskless Engagement in School)

II. School Situational Analysis

These days, majority of QR codes can be read using the camera of a


mobile device, equipped with an application for QR scanning. Once
scanned, they can lead you to a link, a platform or even and application.
More importantly, they can also be a means to relay datasets, including
personal information, depending on the purpose or objective of the one
using it.

The COVID-19 pandemic has pushed many institutions to integrate QR


codes into their existing processes—among them, schools and other
learning institutions. And they have slowly made an impact, whether it be in
the area of teaching or in some other aspect of these institutions’ daily
operations. One of their most popular application in schools has been in
the conduct of contact tracing and contactless gathering of information of
people that have transaction in school.

QR Codes: Cares was developed in Sitio Mata Elementary School to ease


the congestions of crowd in the information desk during retrieval or any
events that is related to school. But it’s also aim to be a used in a long time
period in school for security purposes. By having the QR Code the gathering
of information will be contactless and safer for the school personnel. This
project was launched last April 26, 2021 and been fully distributed last April
30, 2021 for the entire students’ population of Sitio Mata Elementary School.
The recorded distributed QR Code ID was 806 ID out of 806 student’s
population. Over all it was successfully launched and being implemented in
school up to this day. All the stakeholders, parents, and pupils responded well
for this project and gave good feedback.

In the world of technology, the school should also adapt it changes and
embrace the what the world is offering us. I am proud to present to you the
success of PROJECT QR CODE: CARES
III. Applications of the Project

PROJECT QR CODE CARES- QR code is a code that is read by a QR code scanner.


Once scanned, all the information, date, and time of users who visited the school
premises will be recorded. The collected data can be used to identify visitors,
stakeholders, parents, teachers, school staff, and learners who enter school or
even in contact tracing.

No. 14, s. 2020, "Guidelines on the Required Health Standards in Basic Education
Offices and Schools." In line with this, Sitio Mata Elementary School has come up
with the idea of a more innovative way of contact tracing and providing access

for parents in the school facilities at the same time to provide safety and security
for them, as well as the number of people that gain access to school each day
and future face-to-face classes.

Thus, the Project QR Code: CARES was developed for:

1. Easy Access

• Provide easier access and smooth collection of data


• Avoid congestion in the information area
• Monitor efficiency during distribution and retrieval of modules

• Implement the IATF Health protocols and guidelines
3. Safety and Security
• Ensure safety and security of individuals
4. Punctuality
• Instill discipline and punctuality among the learners, teaching and non-
teaching personnel
5. SBM
• Promote best practices to other schools that will lead to a good partnership
to provide innovation for the school development process

On this premise, the Sitio Mata Elementary School proposed and implemented an
innovative project titled QR Code CARES to address these issues, in which 806
parents or guardians, or 100% of the school population, were given ID with QR
Code. We are the only public school that established the coding system in the
Division of Rizal. Since public schools have a limited budget and to make the
project possible, the team came up with an idea that requires a lesser amount
that will be beneficial for public school use. Through the use of the free online
website for QR Code making and Google to gather the data of beneficiaries, we
create. It was launched on April 26, 2021, and the QR Code IDs were fully
distributed on April 30, 2021, for the entire learner population of Sitio Mata
Elementary School. In total, the school issued 806 QR Code IDs to
parents/guardians and stakeholders who had an appointment or transaction on
school grounds. Overall, it was successfully launched and is being implemented
in schools up to this day. All the stakeholders, parents, and learners responded
well to this project and gave good feedback.

IV. Findings

Graphs and their interpretations can be seen below. These are the results
gathered in the survey/feedback form via a Google form. This will be the basis of
the school's continuing the QR code system, supporting the IATF protocol and
easing the congestion in the information area.

The first bar in the bar graph above explains that 75% agreed that the existing and
the use of Project QR Code CARES for getting parents' information are quite good.
The second bar in the bar graph shows that 90% agreed that social distancing
and the safety guidelines in the information area were highly observed. The third
bar in the bar graph shows that 95% of the respondents answered yes, they want
to have a QR code ID. The fourth bar in the bar graph shows that 100% of parents
highly recommend the use of the QR code system in school in terms of data
gathering. The fifth bar in the bar graph shows that 75% of parents answered that
they could recommend this system in other schools. The sixth bar in the bar graph
shows that 78% of parents are willing to give their full information without any
hesitation. The seventh bar in the bar graph shows that 95% of the parents
believed that the use of the QR code system was safe in terms of data gathering.

The positive results of the survey show that Project QR Code Cares was embraced
and produced a positive outcome and benefited the needs of our beneficiaries.

The table reveals that out of 806 learners from Kinder to Grade 6, 550 or 68% of
student population got the QR CODE ID. 68% was considered 100% of school
population because of the rule of QR CODE CARES that one ID per house hold.

V. Recommendations
To help the school planning to continue the program
• That should help the school in the strictly implementation of contact tracing
mandated by IATF
• In providing an easy and a new generation standards of encoding parents
and stakeholders personal information
• To be used as additional coding system in the organization of pupils’ data
and school supply for easy checking or inventory check
• For future use in strengthening the school security for the benefits of pupils
