I, ARCHIE Y. AMPER, a Filipino citizen, of legal age, married, with
residence and postal address at  _________ Davao City 8000, for the
purpose of this Case, Petitioner may be served with summons and
other Court processes, orders and other processes of this Honorable
Court at Carpio & Duterte Lawyers Door 27 Paseo de Roxas
Complex, Roxas Avenue, Davao City, after having been duly sworn
to and in accordance with the law, do hereby depose and say:
 
PRELIMINARY STATEMENT
        That the lawyer asking me questions is ATTY. MARIA
BERNADETTE M. PEPITO conducted the proceedings at her office at
Door 27 Paseo de Roxas Complex, Roxas Avenue, Davao City, and I
am answering the questions fully conscious that I do it under oath
