11. That he/she has not yet complied with the required
units of the Mandatory Continuing Legal Education
(MCLE) since he/she has just been recently admitted
to Philippine bar last ______________ but will be
seeking an MCLE schedule to enroll with anytime and
comply with the units mandated by the Supreme
Court;

16. That he/she undertakes to inform the Executive Judge

within ten (10) days in case of any change in the given
address and to submit his/her reports religiously.

three (3) sample impressions of the Notarial Seal.
