ACCOMPLISHMENT REPORT

I. PROJECT TITLE:
Activity Sheets on “Investigate extent of soil erosion in the community and its effects
on living things and the environment”

II. OBJECTIVES:
 Enhance the learners’ performance in the competency “Investigating Soil Erosion”
 Perform the activities carefully

IV. IMPLEMENTED ACTIVITIES INTERVENTION CONDUCTED

A. Pre-Implementation phase

 The Self Learning Module in “Investigating Soil Erosion” were given to all pupils.
 Identified the students who need improvement and enhancement and those who only need to
complete their missed activities and performances.
 Had Conferenced with the parents/ guardian of these identifies learners.

B. Implementation Phase

 The selected learners will be given contextualized activity sheets in Science V “Investigating
Soil Erosion” and other resources for them to continue learning.
 For pupils who are residing in the community, home visitation is done by the teacher.
 Provide feedback regarding with their assessment results

V. RESULTS:

 Collected and checked all learners’ output.

 Checked through phone conference if the revised and modalities activities were effective
towards improvement of the learner’s performance as shown in their output.
 Observed learner’s enhancements with their learning after the implementation of the
program.

VI. RECOMMENDATION

The proponent proposed to continue this project and hope to advocates continuity of learning
through reading amid covid-19 pandemic. It is suggested that teachers, in collaboration with
parents/guardians, find strategies to bridge the gap
