AFFIDAVIT OF SERVICE

I, Ry An R Ballesta., of legal age, Filipino, with office address at #888

Delbros Avenue, Pascor Drive, Queensway Subdivision, Brgy. Sto Nino,
Paranaque City. after having sworn in accordance with law herby depose and state
that:

1. I am designated as Credit and Collection Assistant of Delbros Waterfront

Chassis Leasing Corporation (DELWATER), a corporation duly organized
and existing in accordance with the laws of the Republic of the Philippines;
2. On June 6, 2017, DELWATER through its Non-Performing Assets served
another Demand Letter but herein defendant refused to receive the demand
letter.;
3. On July 5, 2017, I received the letter sent by Atty Alfredo A. Arungyan III,
counsel for Mr. Tacadao to Atty Patrick Duran external counsel of
DELWATER their willingness to settle their obligation but herein defendant
failed to settle it;
4. On September 19, 2017 , I sent the Demand Letter via registered mail signed
by its Credit and Collection Manager Imelda Sorilla towards herein
defendant;
5. I tried to call Mr. Tacadao in order to negotiate his outstanding obligation
but the latter does not want to settle it;
6. We tried to settle his obligation through Brgy. Conciliation, but herein
defendant refused to accept the Barangay Summon issued by Brgy. Sambag,
Jaro Iloilo
7. Currently herein defendants can be served with summons in their new
business address at San Juan De Coastal, Brgy. San Dionosio, Paranaque
City;
8. I am executing this Affidavit to attest the truth of the foregoing and for all
legal intents and purposes it may serve.
