AFFIDAVIT OF LOSS

I, RAFAEL P. BILLONES, President of Providence Integral Freight Solutions, Inc. Filipino, of legal
age, married, and with residence at Tayud Consolacion, Cebu, Philippines, after being duly sworn to
in accordance with law, hereby depose and state:

1. That I am the President of Providence Integral Freight Solutions, Inc. (Providence Solutions), a
trucking company.
2. That as a trucking company, we have a motor pool of trucks and other vehicles.
3. That as a President, all documents needed for compliances with different government
agencies, including the CERTIFICATE OF REGISTRATION, are in my possession for
safekeeping.
4. That sometime on May 2022, while I was gathering and checking the documents for renewal of
our licenses, franchise, and registration, I noticed that the CERTIFICATE OF REGISTRATION
for one of our PRIME MOVER TRUCKS with plate number ABB-5530 was no were to be found
in my pile of important documents.
5. That despite diligent search and efforts to locate the CERTIFICATE OF REGISTRATION of our
PRIME MOVER TRUCK with plate number ABB-5530, I could not find the same such that I
now have reasons to believe that it is now lost and cannot be recover;
6. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and to
support my application for the issuance of a new CERTIFICATE OF REGISTRATION of our
PRIME MOVER TRUCK with plate number ABB-5530, in lieu of the one that was lost.
