I.

Review of System GENERAL APPEARANCE: Feeling of weakness in the past 7 days with difficulty in concentration especially at school. Mother reports the area around the eyes to be swollen. NEUROLOGICAL Events of headaches and dizziness.

PSYCHOLOGICAL Been more irritable for the past five days due to pain experienced during urination. Difficulty in concentration, mood changes, anxiety, loss of enthusiast in play. INTEGUMENTARY Reports warm skin.

EYES Blurred vision was noted especially upon waking up in the morning.

EARS Patient reported no hearing deficits.

NOSE AND SINUSES Patient had no colds, itching, postnasal drip, sinus pain, nor any obstructions.

MOUTH Patient had tooth decay at 1st and 2ndlower molars.

RESPIRATORY Denied difficulty of breathing, pneumonia, asthma, TB.

CARDIOVASCULAR Denied numbness. Had no reports of chest pain

BREASTS Denied pain, tenderness

GASTROINTESTINAL Regular bowel habit. Has no diarrhea, constipation. Reported incidence of vomiting Reports no incidence of abdominal pain

URINARY Had reported blood in urine, frequency, nocturia (thrice a night), suprapubic pain and bedwetting.

MUSCULOSKELETAL Reported body malaise

MALE REPRODUCTIVE Not circumcised

y GENERAL SURVEY The patient did not have any bouts of fever and chills. Reported fatigability.

NEUROLOGICAL The patient did not report any complaints on headache, loss of movement, change in perception, change in speech, change in smell, loss of memory, tremors, loss of consciousness, and seizures. The patient reports anxiousness.

INTEGUMENTARY The patient doesn t report any rashes. She reported usage of facial cleansers on her face. The patient is very particular with her complexion. She doesn t go for long periods of time under the sun. The patient has dryness of skin, has yellowish skin, and itching.

CARDIOVASCULAR The patient reports no signs of varicosities. The patient doesn t report any palpitations even before. The patient doesn t experience any chest pains. The patient reports no significant changes.

RESPIRATORY The patient has no reports on changes in breathing pattern. No cough and colds is present. She has no shortness of breath. The patient has no cough and colds.

GASTROINTESTINAL The patient reported a change in bowel habit; usually once in three days unlike the once-a-day pattern before. A change on stool color was also noted. The patient noted it as pale brown.

URINARY The patient reported a change in her urine color-darker tea-colored urine. MUSCULOSKELETAL The patient has no reports of significant changes on this part. The patient did not experience muscle weakness.

FEMALE REPRODUCTIVE The patient has five successful pregnancies. All of which were on normal spontaneous delivery. The patient reported that she is taking pills which were recommended by her friend.

ENDOCRINE The patient reports no signs of changes.
