RE: LETTER OF AMENDMENT OF APPLICATION

Dear Ma’am/Sir,

In line with the pending application of my client Mr. Arvin Gaanan, I would like to request for
amendment of the cause of death of his father that has been erroneously entered in his application as
“Muscular Distrophy” and died at 56 years old. However, upon submission of the death certificate of my
client, it was found out that the actual cause of death of his father was due to “Renal Failure” at the age
of 62 years old.
