and appoint any one of the following as my representative

To act for and in my name, place, and stead to perform the following acts, things, and deeds,

(a) To process, transact and secure the Business Permit application of (“the
company”) and its branches/facilities from the Local Government Units where
they are located;

(b) To act as my representative on all matters relating to the foregoing,

including to authority sign, execute, file, and receive any appropriate forms,
requirements, and other relevant documents; and

(c) To perform all acts and things necessary in furtherance of the above
undertakings.

HEREBY GIVING AND GRANTING unto my said representatives full powers and
authority to do and perform all and every act requisite or necessary to carry into effect the
foregoing authority, as fully to all intents and purposes as I might or could lawfully do if
personally present, and hereby ratifying and confirming all that my said representative or
their substitutes shall lawfully do or cause to be done by virtue hereof.
