I am pleased to inform you that you have been offered a place in our institution for industrial
attachment for a period of Three (3) months beginning 11th July 2022 to 30th September 2022
The offer is made to the following terms and conditions:
 You will be based in the Education Section and report to Education Officer who will be
your immediate supervisor
 The attachment is purely on voluntary basis and therefore the institution is not liable to
give any allowances
 You will make your own arrangement for accommodation, transport and lunch
 You will at all times strictly adhere to rules and regulations governing the institution
 You will present a certificate of proof of indemnity from recognized insurance company/
institution
 Your attachment will be terminated in the event of attested case of gross misconduct
 You are required to pay an attachment fee of 3,000/= at the cash office
 You are required to fill in the Industrial Attachment contract forms from the National
Industrial Training Authority and submit it to the institution
If you accept this offer of attachment on this conditions and terms set out above, kindly sign and
return the duplicate for further action
