(Background/Reference Check and other Employment Purposes)

I hereby authorize FactSet Philippines Incorporated and its authorized representatives to conduct a
comprehensive review of my background to be generated for employment purposes. I understand that the
scope of the background or reference check may include, but not limited to the following areas: address,
employment history, compensation details, education background, character references, criminal records
and any other public records.

I further authorize any individual, company, firm or public agency to divulge any and all information, verbal
or written pertaining to me to FactSet Philippines Incorporated. I also authorize the complete release of any
records or data pertaining to me which the individual, company, firm or public agency may have to include
information or data received from other sources. I further authorize FactSet Philippines Incorporated and its
authorized representatives to use the information pertaining to me for job application, employment and
other legal purposes.

In addition, I agree to the release my contact information to FactSet Philippines Corporate Support Services
(CSS) Team so they can provide access credentials to the company network.

I release all persons from liability on account of disclosure. I agree that a photocopy or scanned copy of this
authorization will be accepted with the same authority as the original.
