afetada. Para o diagnóstico clínico de OAJ, é necessário ter dor articular e mais três
dos seguintes achados: mais de 50 anos de idade, menos de 30 minutos de rigidez
matinal, crepitação, alargamento ósseo, sensibilidade à compressão óssea e/ou
ausência de calor à palpação22.
No quadro 1, observa-se a classificação da OAJ femorotibial,37 conforme

Além disso, como as respostas são individuais, pode-se levar algum tempo para que
o manejo da dor seja o mais efetivo25.
Após uma avaliação clínica para identificar possíveis contraindicações
clínicas (cardiológica e reumatológica) que possa descartar acometimentos
articulares de outra natureza, o profissional que irá realizar a prescrição do programa
de exercício físico, deverá realizar uma avaliação física e funcional (Apêndice 1).
Vale salientar que existem questionários específicos para OA, entre eles o
questionário Womac, o qual engloba questões envolvendo dor, rigidez articular e
funcionalidade, onde o maior escore representa a pior condição do indivíduo. Possui
versão traduzida e validada para a língua portuguesa51 (Anexo 1). Outro instrumento
é o Índice Algofuncional de Lequesne (previamente traduzido e validado para língua
portuguesa) que contém 11 questões envolvendo dor, desconforto e limitação de
função, com foco na limitação na dor e distância de caminhada52 (Anexo 2).
Seria interessante que a ficha proposta seja o parâmetro inicial e na medida
em que houver evolução do programa, este seja documentado e apresentado ao
indivíduo, para que sirva de estímulo motivacional. Pode-se ainda, colocar os dados
em planilhas e gerar gráficos para que a evolução seja visualizada de forma mais
concreta.
O início da prática do exercício físico deverá acontecer quando os sintomas
permitirem, com atividades leves e de maneira gradativa. Vale ressaltar, que a
atividade leve poderá ser diferente de uma pessoa para outra. Portanto, a evolução
dentro da estruturação do programa de exercício físico, considerando os níveis
ideais, não é o principal foco e sim atingir o máximo, dentro da possibilidade de cada
indivíduo; reforçando a diretriz onde é mais importante o processo de ser fisicamente
ativo do que o produto de estar em boa forma 5,7.
Os resultados de muitos estudos sugerem que indivíduos com OA, podem
diminuir sua morbidade com a participação regular de atividades físicas
moderadas15. A intensidade do exercício físico pode ser avaliada de várias formas.
Uma delas é a escala de Borg, que utiliza descrições verbais, onde pontua-se de 0 à
10 ( cargas leve, moderada, pesada e máxima);53 e outra, para que se possa ter um
parâmetro para aumentar o nível de atividade gradativamente de maneira mais
simplificada, conforme tabela 6 :

RESUMO
Recomendações e diretrizes têm sido termos cada vez mais comuns em publicações
científicas, demonstrando que o conhecimento não se prende à propriedade de um
autor ou instituição. O rápido acesso à informação necessita do aval de instituições
sérias, éticas e confiáveis, que utilizam de rigorosa metodologia científica,
desprovidas de interesse, senão aquele que resulte em benefício ao indivíduo que
apresente algum tipo de patologia. Diversas publicações sobre tratamentos para
osteoartrite de joelhos (OAJ), provenientes de instituições internacionalmente
reconhecidas, propõem recomendações destacando o exercício físico como principal
abordagem não farmacológica. Por meio de revisão bibliográfica, buscou-se
identificar as recomendações relacionadas ao exercício físico para OAJ, analisar a
estruturação de suas modalidades e prescrição e classificar o nível de evidência e
força de recomendação das abordagens sugeridas. O papel incontestável do
exercício físico na OAJ foi evidenciado em todos os estudos, entretanto, não houve
padronização quanto à estruturação de um programa que contemplasse todas as
modalidades de exercício físico, sendo observado ausência de metodologia
especifica na avaliação dos níveis de evidências e graus de recomendação,
permitindo a inferência da necessidade de estudos que demonstrem de maneira
mais estruturada e abrangente programas de exercício físico que auxiliem a prática
clínica.
ABSTRACT
Recommendations and guidelines have been increasingly common terms in scientific
publications, demonstrating that knowledge does not attach to the property of an
author or institution. The rapid access to information requires the endorsement of
serious, ethical and trustworthy institutions, using a rigorous scientific methodology,
devoid of interest, but one that results in benefit to the individual that presents some
type of pathology. Several publications on treatments for osteoarthritis of the knee
(OAJ), coming from internationally recognized institutions, propose recommendations
highlighting physical exercise as the main non-pharmacological approach. Through a
literature review, we sought to identify the recommendations related to physical
exercise for OAJ, to analyze the structuring of its modalities and prescription and to
classify the level of evidence and strength of recommendation of the suggested
approaches. The indisputable role of physical exercise in OAJ was evidenced in all
studies, however, there was no standardization regarding the structuring of a
program that contemplated all modalities of physical exercise, being observed the
absence of specific methodology in the evaluation of levels of evidence and degrees
of recommendation, allowing the inference of the need for studies that demonstrate a

A partir das explanações contidas nas tabelas acima, observa-se não haver
padronização quanto à estruturação de programas de tratamento e treinamento, tão
pouco encontrou-se estudos que tenham incluído exercícios neuromotores
(equilíbrio e propriocepção), embora se tenha observado similaridades entre as
prescrições.
