10:00:00 AM  Received client from ward with Intravenous Fluid of D5 LRS 1L to run for 8
hours @ around 800cc level and regulated @ 31-32gtts/min
 With complain of flank pain
 With consent for the procedures and anesthesia signed by the patient and
secured to chart.
 Placed with a comfortable position
 Vital sign taken and recorded as follows:
BP – 120/80 mmHg RR – 18 FHT – 150
T – 36.6 PR – 80
10:30:00 AM  IE done by Dr. Amorin and revealed 7-8cm
10:31:00 AM  To Delivery Room Table as ordered by Dr. Amorin
 Patient placed in lithotomy position
10:32:00 AM  Dr. Abrera anestheist notified on duty
10:35:00 AM  Procedure started
10:40:00 AM  Delivered vaginal birth to a live baby boy: APGAR Score 8,9,9
 Placenta completely extracted out
Intravenous Fluid consumed, and replaced by the same plus 10 units of oxytocin to
run for 12 hours
Latest vital signs as follows:
BP – 120/80 mmHg RR – 18
T – 36.7 PR – 79
11:00:00 AM To recovery room
