2) I am bona fide member of Ghatkopar Laxminagar - Sawali Co-
Operative Housing Society Limited, A Co - operative Housing Society,
registered under Registration No. BOM / HSG / 8785 / 1994-95, having
its Office at behind Neelyog Anand Rajaram bane Road, Laxminagar,
Ghatkopar (E), Mumbai - 400 075, hereinafter referred / called as "the
said Society for the brevity's sake. As such owner the Flat I have been
holding (5) Five Shares of ₹. 10 / - Each at face value of ₹.50 / (Rupees
Fifty only) under Shares Distinctive Nos. 386 to 390 vide Share
Certificate No. 78 (Membership No. 78) dated 15/11/2005
3) I say that in pursuant to the member of the said Society, the I have been
acquired government land and erected Zopadi / Flat / Structure /
Tenement lying and being situated at Flat no 906, 9th Floor, A/1,
SAWALI Cooperative Housing Society Ltd, behind Neelyog Anand
RajaramBane Road, Laxminagar, Ghatkopar(East), Mumbai-400075,
The said Zopadi / Structure has been protected by the State Government
of Maharashtra under his various Slum Rehabilitation Scheme world bank
project and carried out Census vide Photo pass Survey Application /
Acknowledgment and issued by MHADA Permanent Photo pass Identity
Card No. under the slum rehabilitation, in lieu of free alternate
accommodation in exchange of my Old Zopadi / Hutment Structure No.
37 and said premise is taken for building construction work vide
Annexure II Serial no.22 Electoral Roll No.1995/246, in Village:
GHATKOPAR lying and being situated on C.T.S no:184(pt), & new CTS
No. 184C & 223 Tal: KURLA have entered into a Triparty Agreement
between Developers M/s. Neelyog Construction Ltd. Company, having
their office at 401/402, Nandprem Shopping Centre, Nehru Road,
Villeparle (E), Mumbai - 400 057 and the said GHATKOPAR
LAXMINAGAR SAWALI Co-operative Housing Society Ltd. under S.R.A.
Scheme dated 11/06/2006 and agreed to allot a Flat / Tenement in the
newly constructed building on free of cost in lieu of free alternate
accommodation in connection with the said Slum Rehabilitation Scheme,
announced by the State Government of Maharashtra, B.H. & A. D. Board
(MHADA) respectively.

4) I say that an (a) Old Electricity Meter No. LT1(B) 7156117, Consumer
No. 151829312, (b) New Electricity Meter No……… ………………………
Consumer No. ……………………., has been installed in the said Flat /
Tenement and paid and cleared all the rent, rates, taxes to the authorities
concerned from time to time, (c) Ration card no. and
Application no. dated
6) I further say that I have full power and absolute authority to transfer
and assign the said Flat / Tenement to the said Transferee. I hereby
declare that henceforth, I, myself or my heirs, or any other person (s)
lawfully or equitably claiming from by, under or in trust for me shall not
claim or have any right, title or interest of whatsoever nature into and in
respect of the said Flat / Tenement.
7) I hereby further state that I have agreed with the transfer to do and
execute and cause to be done and execute all such other and further acts ,
deeds , mattes , and things including attornment of any deeds documents
or writings by and of my relative ( s ) if some lawfully or reasonably
required by the Transferee for the purpose of more particularly, perfectly
and absolutely granting, transferring , assigning and assuring the said
Flat / Tenement to the Transferee or to the name of any other person ( s )
as desired by the Transferee .
8) The Flat has been fully occupied with the consent of all, however in the
future whenever Purchaser/Transferee required any kind of support and

help for the purpose of Flat Registration and other Society or
Registration office correspondence. I MR. NAMDEV ATMARAM MORE
VENDOR / TRANSFEROR (which expression shall unless it be repugnant
to the context or meaning thereof, mean and include her / his/ Their heirs,
executors, administrators and assigns) will be meet whenever required by
Purchaser/Transferee without keeping any request, Expectation or
Condition.
10) I hereby further declare that I agree and undertakes to keep all the
concerned authorities including Ghatkopar Laxmi Nagar - Sawali Co-
operative Housing Society Limited , MHADA ( SRA ) , M.C.G.M. ,
Mahanagar Gas Ltd , Reliance Energy Ltd Tata Power, Rationing
Authorities , and all other concerned authorities indemnified and
harmless against all losses , harm damages and injury arising out of their
acts , deed , matters and things done in connection with the transfer of the
said Flat / Tenement to the name of the said Transferee.

best of my knowledge and belief
