To Whom It May Concern:

This is to authorize the bearer of this note, Ms. XYZ, to transact and perform any action
necessary with the Department of Finance (DOF) in connection with the processing and
procurement of my DOF Clearance and any and all subsidiary documents to be issued
therewith

I further authorize Ms. XYZ to make the necessary payments on my behalf with regard
to the release of my DOF Clearance, as well as to collect my last pay with the DOF after such
Clearance is procured.

Further attached herewith are copies of my Passport ID and my Integrated Bar of the
Philippines (IBP) ID for your reference.
