agent of my husband CHRISTOPHER P. ALONZO, Filipino citizen, of legal age,
married, with postal and residence address at 2261 Unit A, Pasigline Street, Sta. Ana,
Manila, hereby name and constitute Myza Grace Najera or any of his agents, to secure
documents from any government agency or bureau on my behalf such as but not
limited to birth certificates, death certificates, BIR documents, SSS, Philhealth, and
Pagibig documents, etc.
