JOINT AFFIDAVIT OF COHABITATION

WE, HAROLD KING B. HORCA and MARITESS V. MALOLOY-ON, both of legal

age, Filipinos, single and residents of Brgy. Cabadsan, Alangalang, Leyte after having
been duly sworn to in accordance with law, depose and state:
1. That we have been living together as husband and wife under the same roof,
since the year 2007 until this date;
2. That during our cohabitation and even until the present, we remain both of
single status and, hence, there exists no legal impediment for us to marry each
other;
3. That we are blessed with a three (3) children, namely; HENDRIX CLAVINCE M.
HORCA, who was born on August 18, 2008; MEA M. HORCA, who was born on
May 24, 2010; and HESARDO HAROLD M. HORCA, who was born on June 13,
2012;
4. That we are executing this affidavit to attest to the foregoing facts and for the
purposes of contracting marriage without the need of securing the marriage
license pursuant to the provisions of Article 34 of the Family Code of the
Philippines.
