PETITION FOR ISSUANCE OF LETTERS ADMINISTRATION

PETITIONER, through counsel, respectfully alleges:

1. That the Petitioner, who is of legal age and a resident of #156 Purok 4 Santo
Rosario, Iba, Zambales is the widow of CELSO MUSIN DELOS REYES.

2. That on the October 24, 2021, CELSO MUSIN DELOS REYES died
without leaving any will in Iba, Zambales which was his residence at the
time of his death

3. That the names, ages, and residences of the surviving heirs of the
aforementioned deceased, are the following to wit:

5. As far as the petitioner knows, the decedent left no debts.

WHEREFORE, it is respectfully prayed that after due notice, publication and

hearing, letters of administration of the estate of CELSO MUSIN DELOS REYES
be issued to herein petitioner.
