I, Mumtaz Nisar Ahmed Ansari (Aadhar No. 7580 2402 0509), aged about 33 years, an adult
Indian Inhabitant, presently residing at ,do hereby solemnly affirm and declare as under:-

I say that my correctly spelled name is mentioned on my Aadhar card, which is Mumtaz Nisar
Ahmed Ansari (whereas in my S.S.C Marksheet issued by the Mumbai University and Pancard)
its mentioned as Shaikh Mumtaz Begum Nisar Ahmed and Ansari Mumtaz Nisar Ahmed
respectively.

I further say that all the above spelled three names belong to the same person i.e,
myself Mumtaz Nisar Ahmed Ansari.

Whatsoever I have said hereinabove is true and correct to the best of my knowledge and
belief and no material facts have been concealed and/ or fabricated.
