The attached Purchase Order in the amount of thirty eight thousand two hundred one
pesos only (Php 38,201) having been approved, notice is hereby given to AVI OFFICE
AND SCHOOL SUPPLIES TRADING that performance of its contractual obligations shall
commence effective immediately from date of receipt of this Notice on the contract
“PROCUREMENT OF COVID-19 RELATED SUPPLIES”.

Upon receipt of this notice, you are responsible for performing said obligations under the
terms and conditions of the Purchase Order/Agreement.

Please acknowledge receipt and acceptance of this Notice by signing in the space provided
below. Keep one copy and return the original within two (2) calendar days from receipt
hereof to this office.
