TRAINING CONTRACT

I, Marco Pelobello Pielago, BSIT-ELECTRICAL TECHNOLOGY-4 student of Marinduque State

College and enrolled in the Internship Program as an academic requirement of my course, do
hereby enter into a contract with Marinduque State College (MSC) as Higher Education Institution
(HEI) and the _____________________________as Host Training Establishment (HTE) with the
following terms and condition.

1. That I shall comply with all the requirements and policies of the Higher Education
Institution in implementing an Internship subject.
2. That I shall abide with the rules and regulations to be imposed by the Host Training
Establishments while under training in their agency.
3. That I shall exercise due care and diligence in performing tasks assigned to me as student
trainee.
4. That I shall renounce and waive my claim against the Higher Education Institution and
Host Training Establishment for any injury that I may sustain or loss that are beyond their
control.
5. That I shall observe and follow the minimum health safety protocols as issued by the Inter-
Agency Task Force (IATF) during the course of internship.
6. That I shall be made answerable for any liabilities for damage to property or injury to 3rd
parties which may due to my negligence while in the course of my training.
