Afi : “Good morning Ms, I am nurse Afi. Are you family of Mr. Shawn?”
Shinta : “Yes, I am his daughter.”
Afi : “Can I ask you some question to asses his condition?”
Shinta : “Sure!”
Afi : “Can you tell me about his condition and problem?”
Shinta : “He always complain about his back pain when he watering the flowers.”
Afi : “Since when he complains about that?”
Shinta : “Last Tuesday, I think.”
Afi : “Is there any old disease or medication he takes?”
Shinta : “He doesn’t take any medication, but he also got back pain at 2012 and got opname for a
while.”
Afi : “Okay. Can he still hear and see clearly?”
Shinta : “He can hear clearly but he uses glasses for his sight.”
Afi : “Can he do daily activities by himself? Like bathing, eat, go to toilet?”
Shinta : “He can do bathing and toilet by himself, but sometimes he want someone to help him
with his meal and feed him.”
Afi : “Is there any sleep disorder?”
Shinta : “No.”
Afi : “Can you tell me about his sleep activities? I mean the hour.”
Shinta : “He sleep at 9 pm and wake up at 3 am for solat.”
Afi : “Okay, and does he still remember his family and every person on his life?”
Shinta : “No, sometimes he forgot about his child.”
Afi : “How about the route and location in his house?”
Shinta : “He lives with me, and yes he still remember it.”
Afi : “Can you tell me about his personality?”
Shinta : “He is a friendly person and open to anyone! But sometimes I see him looking through
the window blankly.”
Afi : “Okay mam. Thank you for your cooperation.”
