DISCHARGE VOUCHER REQUEST POLICY

ATTENTION

Anne Cohen,

In response to your request as regards the above subject, we are pleased to inform you that you
are eligible for approval provided certain requirements are observed in accordance to Article 59
Subsection 17 of the USARMY-SPS Policy.

To that effect, you have to Purchase a Refundable voucher Service Fee which will be issued by
proponent Agency. Voucher must contain information about paid services as a Security Bond
into the Treasury to respective Camp after duration of Leave requested.

Note: Payment of Voucher is a security Vote and a Bond. Hence, it is refundable back to
you upon your Arrival to the Destination address that you will provide and then your
refund and reimbursements check for the durations of leave will be handed over.

Further Voluntary retirement could also be presented if serving officer is no longer willing to
continue with his service. It is however refundable upon confirmation of arrival to your
destination address and further reimbursements shall be made during the visit.

DURATION FOR LEAVE: 2 months

Voucher Bond Fee: USD-$5540

CATEGORY 3

DURATION FOR LEAVE: 3 months

Voucher Bond Fee: USD $6790
