In September 03, 2021, the data and Internet allowance were received in my Security bank
account.

2. We were mandated to submit a receipt the day after we received the allowance or else, we will
be scolded and reprimanded.

3. With my previous experience, I was reprimanded and being scolded of not able to submit
receipts on time which leads to my mental anxious.

4. So, I decided to falsify my previous data and internet receipt to cope with my mental stress that
I have experienced before.

5. I was not able to purchased Data and internet allowance in the convenience store. At that time,
lockdown restriction was strictly observed due to the rise case number of covid-19 in our Area.

6. I know the company has the right to ask for the receipt of the foresaid allowance but should also
consider the pandemic situation specially when it comes to IATF mandated lockdown.

7. In conclusion, I humbly pray that the company will consider my explanation and accept my
sincere apology.
