Good day!

I owe Keystone Online Solution a sincere apology for submitting falsified data and

internet allowance receipts. I did regret for the act that I did and learn from my mistakes.

In September 03, 2021, the data and Internet allowance were received in my Security

bank account. We were mandated to submit a receipt the day after we received the

allowance or else, we will be reprimanded. With my previous experience, I was scolded of

not able to submit receipts on time. So, I decided to falsify my previous data and internet

receipts to skip of being remanded again.

I was not able to purchased data and internet allowance. At that time, lockdown

restriction was strictly observed due to the rise case number of covid-19 in our area. I know

the company has the right to ask for the receipts of the foresaid allowance but I was not able

to go to the main city (Iligan City) because of the pandemic situation. There are check points

that requires legal papers and only authorize person can go to the main City as mandated

by IATF.

I humbly pray that the company will consider my explanation and accept my sincere

apology. Please forgive me and provide another chance. I will never do it again. I learned

many things and reflected on what I had done wrong.
