proceed with their initial registration. On the actual appointment date, you shall bring and
submit a copy of your Oath taking for Professionals Form and will then be instructed to
sign in the Commission Registry Sheet. After successful initial registration as a new
professional, you will be issued your PRC Identification Card (PIC). For further
