Consolacion Maritess De la Vega known as “Lola Macon” Born sept 18, 1946, married, retired
mechanical engineer in an international company founded in London. She said she was very busy when
she was still working in London because her company caters to a lot of construction centers not only in
London but also in some countries. Time is gold for her. She used to move from one branch to another.
5 years ago, she was diagnosed with Parkinson’s disease and complain of heart palpitations at times,
she’s in the geriatric facility for 7 years now. She had 5 children, all of them are abroad, her husband
died 10 years ago of CVA for he was a heavy smoker and drinker as well. She said her father died of
cancer while her mother-in-law was treated for tb. She was placed in geriatric facility by her children
because she is living alone in their house. Lola Macon has a stooped posture, wrinkled skin, gray hair,
slow hearing ability and wearing of eyeglasses for her eyesight is quite poor.
She has noticeable tremors or shaky hands; she walks with a cane. She asks for assistance for her daily
hygiene care needs. She had her check-up done yesterday due to on and off fever and was given
paracetamol 300mg tab RTC for fever. She is still waiting for her cbc, blood chem, chest x-ray results.
Her maintenance medications include Micardis 40mg once a day, Sinemet 75 mg once a day, Vasalat 25
mg once a day at bedtime and Metformin 500 mg at bedtime. She said that she takes her maintenance
medications religiously although she is usually reminded by the nurses daily.

Lola Macon loves chicken and pork for her foods especially on lunch and dinner time, she usually asks
for white tea and bread with egg for breakfast. She added that her motto “sanaol yolo”, enjoy while still
alive, pray the rosary daily to give thanks for reaching her age and chika with her room mates and of
course eat sweet foods. She usually joins the mass and received the holy sacrament of the mass
rendered by the facility daily. She watches news at early evening and usually sleeps at around 8pm and
wakes up at 5am and walks slowly to the veranda early morning for early fresh air where she will pray
her morning thanksgiving to God. She usually joins the activity done by the facilitator of the facility for
morning exercise and therapy daily. She said she misses her children more frequently maybe that’s why
she’s having trouble sleeping lately.

1. What is geriatric functional assessment tool?

The geriatric functional assessment tool is an instrument used to obtain an elderly patient’s data
about health, functional ability, and ability to live independently. It is used by a trained health
multidisciplinary team comprising of a doctor, nurse, and physiotherapist in doing a
comprehensive geriatric assessment. The involvement of an occupational therapist, psychologist,
and social worker is optional.

2. What is the importance of functional assessment in geriatric patient?

The functional assessment in geriatric patients can decrease the number of medical specialist
visits and overall health care costs. It can increase patient satisfaction and quality of life. It also
determines the current and future health care and psychosocial needs of elderly patients.

3. What is the goal of geriatric assessment?

The geriatric assessment aims to assess an elderly’s health and functional ability, rehabilitation,
and ability to live independently. It aims to identify the factors that weaken the functional ability
of the patient and to make the treatment and rehabilitation plan.

4. Why functional assessment is used when assessing elderly?

A functional assessment is used when assessing the elderly because they are at high risk of
having multiple health problems and difficulty carrying out their activities of daily living. It is
used to assess and quantify an older adult’s functional status, where this information is used by
the health care team to develop a comprehensive plan for treatment and future care decisions.
