WE, RAMEL B. PEDIDA and JOHNNY B. YBANEZ, we both of legal ages, Filipino, single,
and with residence addresses both at Divisoria, Cataingan, Masbate, after having been duly sworn to in
accordance with law do hereby depose and state, that:

1. We personally know JOSE RAMEL T. CENIZA of Brgy. Matayum, Cataingan, who was
born on April 21, 1971 to spouses Atanasio A. Ceniza and Pastora B. Temblor;

2. That Jose Romel T. Ceniza, as registered in his baptismal certificate, and Jose Ramel T.
Ceniza, as registered in his live birth, is one and the same person;

3. That Jose Ramel T. Ceniza, was not able to graduate in his elementary education;

4. That we are not related by any degree of consanguinity or affinity JOSE RAMEL T. CENIZA
or any member of her family.
