Day 1
F- Impaired social interaction.
D- Received sitting on bed, talked immediately upon approaching him, has
racing speech, loud voice, appears restless, has poor attention span and
difficulty focusing on one thing at a time, mood swings.
A- Noted negative behaviors observed previously by caregivers, coworkers, and
family. Provided a quiet environment, Active listening and provided client a safe
environment. Provided positive reinforcement for improvement in social
behaviors and social interactions. Encouraged client to participate in
developing positive social skills, encouraged to engage in solitary activities,
encouraged ongoing family or individual therapy as long as it is promoting
growth and positive change. Encouraged patient to verbalize feelings.
R- Was able to communicate with others without being impulsive and
manipulative. Endorsed

Day 2
F- Self-care Deficit
D- Received lying on bed, “Ayaw ko kumain”, unkempt hair and still haven’t
groomed, appears restless, irritable, easily distracted
A- Recognized client’s choices for food, personal care items, and other things;
monitored impulsive behaviors or actions indicative of altered judgment;
assisted in using toiletries and hygiene aids; gave frequent encouragement and
aid with dressing as needed; gently instructed client using a step-by-step
method; educated the patient’s caregiver on providing appropriate assistance to
the patient while he performs self-care tasks.
R – Patient was able to dress and groom himself in appropriate manner.

DAY 3
F- Diarrhea related to anxiety
D- Received lying on bed crouching, “Pangatong balik ko na to sa CR, hindi na
nakakatuwa”, report of cramping in the abdomen, has loose stool, urgency to
go to the toilet,
A- Assessed for abdominal discomfort, evaluated the pattern of defecation,
assessed stress levels, Gave antidiarrheal drugs as ordered, promoted the use
of relaxation techniques to decrease anxiety, allowed patient to use free time to
relax, encouraged intake of fluid, explained the need to avoid stimulants,
encouraged to eat high fiber food, explained possible cause of diarrhea.
R- Patient used relaxation techniques. Endorsed.
