AFFIDAVIT OF DELAY

I, A2C RANDY S. ESTRERA JR 927890 PAF, of legal age, married and

enlisted personnel of MILITARY INTELLIGENCE GROUP 12, INTELLIGENCE SERVICE
ARMED FORCES OF THE PHILIPPINES after having been sworn to in accordance with
law, do hereby depose and say:

That, I submitted the request for my 1st Longevity Pay to HCTIF, ISAFP on 04
October 2021 that was prior year of the affectivity of my 1st Longevity Pay.

That I was not informed by the Admin Personnel of CTIF that I have lacking
documents to be submitted to Personnel Division.

That I am executing this Affidavit of Delay to attest the truth of the foregoing.
