PRELIM EXAM/ REQUIREMENTS

Instructions:
Submit the soft-copy of the following documents to marohom11ayno@gmail.com.
You may use Google or other related sites as a guide, but DO NOT “COPY AND PASTE” from
the internet.

1. Make a Courtesy Call letter address to the Punong Barangay. The letter should
contain your intent to conduct a Community Organizing/ Study to the said
barangay.
2. Submit a request letter to the Punong Barangay to secure the copies of (1)
Demographic/ Barangay Profile, (2) Health Center Records and (3) the barangay
Spot Map.
3. Make your own Community Survey form. Suppose you are to conduct a house-to-
house survey for your CO, how would you make your comprehensive survey form?
You may use the DSWD’s Social Welfare Development Indicator (SWDI) Form as an
example/ reference. Also include your own version of a General Intake Sheet.
4. As a proof of your community engagement, submit your scanned Certificate of
Appearance from the barangay.
5. And lastly, submit a copy of your “Minutes of the Meetings” captured from your
first team meeting.
