3 For each appropriate sheet, please insert the region's logo and indicate the name of the region in the heading;

5 For nominated schools which are fully compliant with SSAT, all fields must be accomplished;
6 For schools which are SSAT compliant but with pending LGU concurrence, encoders may leave other fields blank except School ID, School Name and SSAT Complia

DEFINITION OF TERMS
Pilot School - refers to the school which was included in the initial pilot implementation of
the face-to-face classes which commenced on November 15, 2021
Additional Pilot School - refers to the school which was included in the expansion of the
pilot implementation of the face-to-face classes which commenced on December 6, 2021
Progressive Expansion - refers to the schools which will be included in the expansion
phase of the limited face-to-face classes in 2022

SPECIFIC INSTRUCTIONS

Input the inclusive date when the school was approved to start the conduct of the implementation of limited face-to-face classes.

Input the total number of learners participating in the entire duration of limited face-to-face classes in the school per grade level,
according to appropriate columns.
Input the total number of teachers participating in the limited face-to-face classes in the school.
Input the total number of non-teaching personnel participating in the limited face-to-face classes in the school.
Under each of the corresponding learning platform, select "YES" if the appropriate learning platform is being implemented by the
school; select 'NO" if not. The encoder may select more than one (1) learning platform.
