Dear Ms. Enriquez:

The attached Contract having been approved, notice is hereby given to SEQUOIA
HOTEL MANILA BAY that service may commence for Supply and Delivery of Food,
Venue and Accommodation in Metro Manila on November 24-25, 2022 effective
seven (7) days upon issuance of this notice.

Upon receipt of this notice, you are responsible for performing the services under the
terms and conditions of the Agreement and in accordance with the Implementing
Schedule.

Please acknowledge receipt and acceptance of this notice by signing both copies in the
space provided below. Keep one copy and return the other to the Department of
Agriculture, Region IV-MIMAROPA 3rd Floor ATI Bldg., Elliptical Road, Diliman, Quezon
City.
