Letter of Request

I, Jennifer T. Dacdac, is writing to your good office for the request of cancellation of my WIFI
installation account numbered as 1151502034195. Also, I am asking for the refund of the initial
payment I have paid to your office amounting to PHP 1,500.00. I enclosed in this letter my valid
ID. I also want to note that I appointed and authorized my cousin, Graciela L. Inacay, in
processing this matter and to receive the said amount. I do hope that this matter will be settled
