INFORMATION

The undersigned Assistant City Prosecutor, upon sworn complaint originally filed by the
offended party, accuses_______________ of the crime of Attempted Bribery, committed as
follows:

That on or about_______________ in the city of_____________, province

of_______________ , Philippines, within the jurisdiction of this court, the
said_______________ , did then and there, willfully, unlawfully, feloniously and maliciously
promised, offered, and delivered to_________________ , a member of the Philippine National
Police stationed in _____________ , duly-appointed, qualified and acting as such, the amount of
_________________________ to the end that said police officer would refrain from complying
with his official duties as such police officer, that is, to refrain from arresting the said accused
and from filing charges against him for violation of the_______________ , an act committed in
the presence of the police officer, but the said accused could not carry out his purpose to corrupt
the said officer by reason of causes independent of his own voluntary desistance, because the
said patrolman did not allow himself to be corrupted nor was he willing to abstain from the
lawful compliance of his official duties as such officer.
