INFORMATION

The undersigned Assistant City Prosecutor, upon sworn complaint originally filed by the
offended party, accuses _____________________ of the crime of RAPE, committed as follows:

That on or about__________________ in the city of____________, province of

_______________ , Philippines, within the jurisdiction of this court, the said accused, being then
and there members of the Philippine National Police stationed a t_________________, and
having individually ad collectively conceived the idea of revenge upon the police force of said
___________________ for certain alleged grievances, conspiring together and confederating
themselves, did then and there willfully, unlawfully and feloniously leave the said barracks
without roper authority, and rise publicly and tumultuously in order to attain by force or other
means outside legal methods, the object of inflicting an act of hate and revenge upon the persons
of the police force, as in fact the said accused did inflict by then and there firing and shooting
members of said police force indiscriminately at several places of said ___________________ .
