NEUTROPHILIC DERMATOSES IN AN EIGHT YEARS AND TEN MONTHS OLD BOY: A

CASE REPORT

OBJECTIVE: Neutrophilic dermatoses (ND) is a heterogeneous group of skin diseases


characterized by histologic evidence of infiltration of the skin by normal mature neutrophils
without evidence of overt infection. Several subtypes of ND have been suggested, including
Sweet Syndrome, ND of the dorsal hands, and gangrenous pyoderma. Cutaneous findings vary
and many ND patients have overlapping features. The aim of this study is to report ND in an 8
years and 10 months old boy.

CASE: An 8 year and 10 month old boy was hospitalized with chief complain of indurated
erythema on the face, body, arms, back and thighs without pain and itching since 1 month prior
to admission. Physical examination showed some nodules on the left lateral nasal, left
maxillary, right and left lumbar, right and left posterior antebrachii, right and left anterior femurs
region. Laboratory examination revealed anemia, leukopenia, elevated liver enzymes, CRP and
ASTO. At first, patient was diagnosed and treated with rheumatic fever, but after 2 months
treatment there was no recovery of skin lesion. Skin biopsy was taken and histopathological
findings demonstrated neutrophilic inflammation extending from the superior dermis, deep
dermis to subcutis. The patient was treated with steroids and achieving skin lesion recovery
after several weeks.

CONCLUSION: Neutrophilic dermatoses skin diseases with cutaneous findings vary and many
patients have overlapping features. Histopathological features were characterized by neutrophil
predominantly infiltrates. The case was promptly cure clinically after administrating
corticosteroids.
