Science High School, would like to humbly request for the following in connection with the
upcoming End of School Year (EOSY) Rites.
1. Face to Face rehearsal on June 27, 2022, and Moving-up Ceremony at the
Metropolitan Theatre, Ermita, Manila on June 28, 2022, to be attended by both
parents/guardians of the Candidate for Completion.
2. Wearing of Sablay during the said ceremony as requested through the effort of MSHS
GPTA.

I/We hereby allow our child’s name, photos, and videos to be taken and released on
different media platforms including but not limited to streaming services and other social media.

We fully support implementing all applicable health protocols in compliance with the
government authorities’ prescription, including wearing of face mask at all times, seating at the
designated area, and observing safe distance.

In connection, I/We hereby undertake to render free from any claim or liability any
officer/employee/representative of the Schools Division Office-Manila, the MSHS
administration and faculty, the MSHS General Parents-Teachers Association, and the members
of the Grade 10 Moving-Up Committee in relation to the participation of our child to above-
mentioned activities.
