Found out the patient was taking 2.5mg bisoprolol instead of the prescribed dose(1.25), in the morning started on
the prescribed dose
14.5.2022
Returned to taking 2.5mg bisoprolol
Irregularly irregular pulse palpated and auscultated in the morning
ER visit → AF started on apixiban 5mg x 2, Plavix reduced to 75mg x 1, aspirin omitted.
By noon, the AF had resolved and pulse is regular again

15.5.2022
Cardiologist follow up:
He reported echo is much better than 1 week ago, no akinesia in walls
He prescribed entresto 25mg x 1 and advised monitoring BP due to suspected falls after starting entresto
Kidney function tests and serumlytes in 1 week

16.5.2022
Bigeminus pulse in the morning, normal and stable vitals
Resolved in midday and reappeared at noon, stable vitals, only complained of “skip beat fall” in the night
