participate in the research study on “Attitude of Nurses toward the use of Electronic Health
Records”.

INFORMATION SHEET

I am Jenisha Sapkota, BSc. in Nursing 15th batch student at Kathmandu University School of
Medical Sciences. I am doing my research on “Attitude of nurses toward the use of electronic
health records”.

Purpose of the study: In the 21st century, we have started storing patient information in
digitalized electronic health recording format. Nurses have to interact with electronic health
records software like Mediflow on a daily basis. I want to assess the attitude that nurses hold
towards use of electronic health records. I believe you can help us to gain more insight in this
topic by answering how you feel about the electronic health records.

Procedure: You will have to fill out a 20 items questionnaire. It will only take around 10
minutes of time.

Risks: The study doesn’t pose any threats. If you feel like you may share your personal or
confidential information by chance, you don’t have to answer them.

Benefits: There will be no direct benefit to you but your response will help us to find out more
on the attitude of nurses towards the use of electronic health records.

Principle of Autonomy: Your participation in this research is entirely voluntary.

Right to refuse and withdraw: You can refuse to take part in this study or withdraw at any time
if you want.

Confidentiality and anonymity: Your information will be kept confidential and won’t be
shared for any other purpose. Any information about you will have numbers instead of names
and your information will be stored securely.
