AFFIDAVIT OF DISCREPANCY
I, RODANTE D. MENCIOF, of legal age, Filipino, single, residing at Baguio City, after
having been duly sworn to in accordance with law, hereby depose and state:

1. That I was born on APRIL 16, 1998, in Kiangan Ifugao;

2. That when my parents enrolled me for my formal education, they registered and
entered RODANTE D. MENCIOF as my appellation;

3. That in all my private and public undertakings I entered and supplied my name as
RODANTE D. MENCIOF;

4. That when I secured my Live Birth Certificate with the Philippine Statistics
Authority (former National Statistics Office) I discovered that my complete name registered
and entered with said office was RODENTE D. MENCIOF;

4. That the names “RODANTE D. MENCIOF”, and “RODENTE D. MENCIOF” refer to

me and to me alone born on APRIL 16, 1998;

5. That the discrepancy of entries of my names on my school records that registered

and entered with the Philippine Statistics Authority (PSA) was due to inadvertence, done in
good faith and did not arise from malevolence or sinister motive or done to hide my
identity or to defraud any third party;

6. That I am executing this Affidavit to attest to the truthfulness of the foregoing and
for all legal intents and purposes.
