    67 yr old
    Post menopausal lady
    Resident of vadodra
    Chief complain

    Complain of spotting per vaginum on and off for 7


    months
    2-3 episodes
    HOPI

     Patient was apparently alright 7 months back then she developed


    complain of spotting per vaginum on and off . 3-5 episodes staining
    only under garments and not needing pads
     Not associated with any foul swelling discharge
     No history of any hormonal drug intake or topical application or anticoagulants
     No history of any vaginal pessary use
     No history of genital trauma or ulcer in private parts or any surgery
     No history of blood in urine, burning micturition, increase urinary frequency (UTI,
    urethritis)
     No history of bleeding per rectum
     No history of pain during defecation
     No history of abdominal distension
     No history of nausea vomiting ad constipation
     No history of easy fatiguability
     No history of decreased appitite and weight loss
     No history of any lump in breast or lump in abdomen
    MENSTRUAL HISTORY:
     Menarche 12 yrs
     Menopause 56
     Post menopausal since 10 years
     Previous cycle was normal regular 28-30 d cycle
     3-4 days normal flow 2-4 pads per day
    OBSTRETIC HISTORY

     Married 40 yrs back


     Nullipara
     No history of contraceptive failure
    PAST HISTORY:

     Known case of hypertension since 20 years on oral drugs tab


    amlodipine 5mg od
     Known case of diabetes mellitus on oral drugs gluconorm 500 mg od
    ..sugar under control
     No other medical or surgical illness
    FAMILY HISTORY

     NOT SIGNIFICANT
    PERSONAL HISTORY

     Diet vegetarian
     Sleep wake cycle normal
     Bowel /bladder normal
     No history of allergy or addiction
     No history cervical canal surgery
    General examination

     Patient conscious well oriented with time person place


     Heavy built
     Bmi-32
     No pallor cyanosis clubbing lymphadenopathy’
     No odema
     Pulse -82 /min in right radial artery with good volume normal character no
    radiofemoral delay all peripheral pulses are present
     Blood pressure – 130/86 mmhg in rt arm in sitting position repeated after 15 mins
     RR-16 /min
     Temp –afebrile
     Jvp not raised
     Bilateral breast normal
    Systemic examination

     Cvs- apex beat 4th intercostal space in left anterior axillary lines1 s2
    normal no murmurs
     Respiratory system- air entry bilaterally equal no additional sounds
    heard
     Cns- no sensory /motor deficits. All reflexes normal
    Local examination

     External genitilia: vulva vagina healthy


     Urethral opening normal
     Anal opening normal
    Per speculum

     Cervix and vagina healthy


     No atrophic changes in vagina
    Per vaginum examination

     Cervix- down and forward, smooth


     No atrophic changes in vagina
     Uterus- anteverted 6 wk size uniformly enlarged
     Firm in consistency non tender mobile
     Bilateral fornices free non tender mobile
    P/V/R

     Rectal mucosa free


     Bilateral parametrium free
     No nodularity in pouch of douglas
    Provisional diagnosis

     67 yr old nullipara woman with post menopausal bleeding with


    grade 1 obesity , hypertension and diabetes probably due to
    endometrial cause.
