AFFIDAVIT OF LOT OWNERS CONSENT

That I, RICARDO E. QUIRANTE, of legal age, married, Filipino Citizen and a
resident of Brgy. Inlambo Mangaldan, Pangasinan, after having been duly sworn to in
accordance with law, do hereby under oath deposes and says:

1. That I am the heir of late SPS. POLICARPIO QUIRANTE and

FELICIDAD QUIRANTE, registered owner of parcel of Residential land
located at Brgy. Inlambo Mangaldan, Pangasinan, declared by Tax
Declaration No. 25-0016-00489;

2. That in the portion of the above portion of land is a constructed sari-sari

store by my sister, LUCRECIA Q. VINLUAN;

3. That I am giving my full consent to my sister LUCRECIA Q. VINLUAN to

apply for Electrical Permit and other necessary permit for the constructed
house;

4. That I am executing this affidavit to attest to the truthfulness of the

foregoing facts and statements and for all legal intents and purposes.

located at Brgy. Inlambo Mangaldan, Pangasinan, declared by Tax
Declaration No. 25-0016-00489;

2. That it is my desire to apply for electrical permit on my constructed

sari-sari store in the said residential house;

3. That I undertake and bind myself to submit all the requirements needed
for the application;

4. That I am executing this affidavit to attest to the truthfulness of the

foregoing facts and statements and for all legal intents and purposes
without any purpose of deceit or misrepresentation.
