AFFIDAVIT OF TWO DISINTERESTED PERSONS

 
          We, THELMA L. BACAMANTE, of legal age, married, Filipino and a resident
of Bagacay, Dumaguete City, Negros Oriental, Philippines, and JULITA DIEZ
MONTANO, of legal age, married, Filipino and a resident of Camanjac, Dumaguete
City, Negros Oriental, Philippines, after having been duly sworn to in accordance with
law freely depose and say;

1. That we know personally the person of MARIA VICTORIA Z. TIGAS, being our
friend;

2. That her maiden name is MARIA VICTORIA P. ZAMORA and after her
marriage, she used the name MARIA VICTORIA Z. TIGAS;

3. That because of typographical error, it appeared in the CERTIFICATE OF

EMPLOYMENT and Employment ID issued by Whynot Resto Bar that her name
entered as “MARIA VICTORIA P. ZAMORA” instead of “MARIA VICTORIA Z.
TIGAS”;

4. That her true and correct name is MARIA VICTORIA Z. TIGAS;

5. That the names MARIA VICTORIA P. ZAMORA and MARIA VICTORIA Z.

TIGAS belong to one and the same person for all legal intents and purposes;

6. That in view of the prevailing discrepancy, we were constrained to execute this

affidavit to attest to the truth or veracity of the facts herein above-stated and
that this affidavit is executed without the taint of any undue or improper
pressure and influence exerted upon our mind and body.

IN WITNESS THEREOF, we have hereunto set our hands this April 4, 2022 at
Dumaguete City, Negros Oriental, Philippines.
